As a new client, you have the opportunity to fill out all forms prior to your initial appointment through our Patient Login but you will need a security code to do so. To receive a personalized code, please call our office at 323-464-8046. Should you not wish to complete forms prior to your scheduled appointment, you should arrive 15 minutes prior to your scheduled consultation to complete a client profile.
When calling, please have a credit card number available, so we may hold your scheduled appointment time. We will not run any charges at this time and you are welcome to pay for your appointment with another card or method of payment.
While cosmetic consultations are not complimentary, the consultation fee will be waived should you wish to have treatment during your consultation appointment. Please call for consult fee information.
The office of Rebecca Fitzgerald, MD accepts American Express, Care Credit (based on approval), Discover, MasterCard, Visa, personal checks, and cash. We do not offer payment plans other than CareCredit Financing, which you can apply for through our office at the time of your appointment or on the CareCredit Website at any time.
24-hour advance notice is required to reschedule or cancel any appointment. Failure to do so or not show up at an appointment without a phone call will result in a missed appointment/late cancelation fee.
ALL products, whether opened or unopened, are non-refundable. Unopened products may be exchanged within 14 days of purchase for product credit only. Opened products and prescription items (Latisse, Cosmelan 2, Obagi) are not returnable.
To purchase prescription grade products on our website store, you must be a current patient of the office.
If at any time your contact information or insurance coverage changes, please notify our office at your earliest convenience.
